Strengthening GP Relationships Through Community Partnership Management

Strong community healthcare isn’t just about delivering clinical services, it’s about building trusted, responsive relationships with the GP practices we support. That’s why our Community Partnership Managers play such an important role in how we work with primary care.

As a provider of NHS-commissioned community services, we understand the pressures GP practices face every day, from managing referrals and patient expectations to navigating system-wide capacity challenges. Our Community Partnership Managers are there to provide a named, reliable point of contact, helping practices feel supported beyond the referral itself.

Honest feedback, real-world experience
Feedback from GP practices helps us understand what’s working well and where additional support makes a difference. One GP recently shared their experience of working with HealthHarmonie, highlighting both the value of the services and the importance of having a dedicated contact:
