As a provider of NHS-commissioned community services, we understand the pressures GP practices face every day, from managing referrals and patient expectations to navigating system-wide capacity challenges. Our Community Partnership Managers are there to provide a named, reliable point of contact, helping practices feel supported beyond the referral itself.
